Mental Health Case Manager, Community Response Team

WORKSITE LOCATION: Gulf Bend Regional Plaza, 6502 Nursery Drive Suite 100, Victoria TX 77904

PAY GRADE: 14 EEOC CODE: Professional FLSA STATUS: Non-Exempt

SHIFT: Fulltime; rotating shifts M-Th or T-F, 8am-7pm

EDUCATION AND EXPERIENCE REQUIRED: Bachelor's Degree from an accredited college/university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special educatoin, educational psycholoty, early childhood education, or early childhood intervention -or- is a registered nurse -or- is a Licensed Practitioner of the Healing Arts (LPHA).

Preferred: Work experience in mental health preferred; experience working within jail setting.


REPORTS TO: Team Lead, Community Response Team



This position will work as a team member of the Community Response Team (CRT). Will serve as an advocate for persons throughout our GBC Region and coordinate programs of support throughout the community for the benefit of persons who suffer from mental illness. Will provide crisis intervention services as needed to reduce symptoms with the intent to prevent admission to a more restrictive environment. Will provide case management services to link to appropriate community resources and services to prevent incarceration and hospitalization. Will work in cooperation with family members and community organizations including, but not limited to law enforcement, hospital staff and other community providers. Will assist with and/or complete emergency warrants as necessary. Will provide services from within our 7- county service area.


  1. Responds to community calls from dispatch and/or crisis hotline.

  2. Links clients and/or their families to appropriate community resources.

  3. Thoroughly completes a crisis log with supporting documentation.

  4. Provides crisis services per HHSC contract, Medicaid Rule, and Center policy.

  5. Completes all paperwork according to Medicaid Rule, HHSC Contract and Center Policy.

  6. Demonstrates appropriate responsiveness to consumer needs as demonstrated by no legitimate consumer, collateral or co-worker complaints.

  7. Submits documentation of services and data tracking sheet within two business days.

  8. Completes CSSR-S and Safety plan.

  9. Attends all required team meetings as scheduled by supervisor unless excused by supervisor.

  10. Demonstrates an ability to communicate effectively both orally and in writing 100% of the time.

  11. Uses a computer and/or other technical devices to complete all work assignments.

  12. Completes fleet checklist as required.

  13. Demonstrates efficient time management as evidenced by meeting all job expectations.

  14. Must maintain regular and reliable attendance.

  15. Complies with telework/remote agreement.

  16. Uses the trauma informed care approach when providing services and working with others.
  • Understands the Three E's of trauma-informed care.
  • Understands the Four R's of trauma-informed care.
  • Understands the six key principles of trauma-informed care.

A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

  1. Computer, telephone, calculator, computer, fax machine, and copy machine are used in performance of essential job duties.

  2. Various forms of paperwork are handled in performance of essential job duties.

PHYSICAL REQUIREMENTS and ENVIRONMENTAL CHECKLIST: Abilities required to perform the essential and marginal tasks of this job: Walking, operating office equipment, standing, operating motor vehicle, light lifting (under 15 lbs), sitting, ability to see, identify colors, light carrying (under 15 lbs), hearing (with aid), straight pulling, ability to write, ability to count, ability to read, simple grasping, ability to tell time.

Work site and environmental conditions that apply to this job: Travel by car 50% of time, outside, inside, working closely with others, working alone.



  1. Interacts with employees' contractors, customers, community, family members, referral sources, governmental sponsoring and funding entities, program auditors, advocacy groups, and the media.

  2. Ability to communicate effectively, both verbally and in writing.

  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  • Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  • Good driving record as defined by Gulf Bend Center.
  • Must complete all CORE training requirements prior to the due date.
  • Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  • Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
