Job Description:
To provide support to the Support Manager and Area Director by
assisting in employment processes, scheduling, quality assurance,
compliance with all program principles, and ensuring a safe and
healthy environment for consumers and staff.Essential Functions
- Provides personal assistance with daily living activities of
consumers (grooming, eating, personal hygiene, etc.).
- Reinforcement of counseling and therapy activities of consumers
as directed by the PDP.
- Insures health and safety of all consumers and Team Members by
identifying and correcting safety hazards.
- Provides assistance with medications and performs tasks
delegated by a Registered Nurse
- Ensures programmatic needs of individuals are met by
observation of direct care team members and daily review of
programmatic documentation. Responsible for ensuring direct care
team members follow PDPs as written and provides coaching and
counseling when needed. Reports concerns with programs to Support
Manager.
- Works with Support Manager to coordinate coverage and provides
coverage if no other provisions can be made.
- Uses resources efficiently. These resources include, but are
not limited to, costs associated with vehicle maintenance, usage
and overtime.
- Completes the interview process for potential team members and
hires new team members including completion of paperwork and
ensuring new hires complete orientation prior to on-thejob
training. (Orientation and nursing delegation is scheduled by
supervisor.)
- Provides on-the-job training for new team members according to
policy and procedure and support for others in areas where
assistance may be needed.
- Attend all required meetings/enrollments of Texas Home Living
individuals and HCS individuals.
- Completes paperwork including Service delivery logs, data
collection sheets, timesheets and ensures timely submission of all
paperwork in Daybreak Billing Dept.
- Performs all other duties as assigned by management.Supervisory
Responsibilities:
- Will interview, hire, train provide supervisory guidance and
resolve personnel issues of the Direct Support Team members with
the direction of the Assistant Area director and Human Resources in
accordance with all policies and procedures.
- Always ensures adequate staff coverage.
- Trains all Direct Support Team members on the philosophy and
policies and procedures of Daybreak.
- Conducts regular evaluations of direct support Team Members to
ensure that team members are aware of performance expectations and
are meeting those expectations.
- Will ensure corrective action process is followed if Team
Members fail to meet required expectations or if Company policies
or procedures are violated.Education and Experience:
- Bachelor's degree or equivalent experience
- Minimum of 2 years working with individuals with developmental
disabilitiesQualifications:
- Good communication skills
- Ability to handle multiple tasks efficiently and effectively
- Valid Texas Driver's license and good driving record
- Ability to perform all functions as stated in the residential
support provider job description.
- Knowledge of HCS and TxHmL program
