Full-time Description Job Title: Red Cord Case Manager Classification: Full Time/Non Exempt Reports to: Director of Community Health Outreach Salary: $35,000 - $40,000 Summary: The Red Cord Case Manager provides intensive, trauma-informed, and culturally responsive support services to individuals experiencing all forms of commercial sexual exploitation and/or labor trafficking. This is a temporary position, funded through September 2026. Responsibilities: Dedicated to building continued collaborations with organizations to ensure individuals are properly identified and referred for appropriate housing and services through Red Cord. Assists in the development and implementation of individual care/service plan that promotes positive behavior change, victim safety planning, and connects clients to appropriate resources. Engaging clients through intensive case management services which involves building a trusting relationship with the client and providing on-going support to help the client function in the least restrictive, most natural environment and achieve an improved quality of life. Navigate emergency shelter and transitional housing for clients in need of housing support. Care coordination to help clients integrate into the community by connecting individuals and their dependents with services such as transportation, counseling, childcare services, medical care, employment, and other social support. Track client progress, identify challenges, and making adjustments to the care plan in collaboration with the client as needed. Conduct proactive street outreach to engage with high-risk populations, promote protective measures and connect individuals to supportive services Participate in community education events, presentations, and trainings to raise awareness on commercial sexual exploitation and human trafficking. On-Call crisis response – staff rotate coverage to provide screening, assessments, crisis intervention, and supportive care to clients for evening, night, weekend, and holiday on-call duty. Requirements Qualifications: Must have current and valid Driver’s License and the ability to be insured Bachelor of Arts or Science degree in a social, behavioral, or human service field Bilingual (English & Spanish) required. Experience/Skills/Knowledge: Strong interpersonal and communication skills and the ability to work effectively with a wide range of populations in a diverse community. Ability to initiate and maintain positive working relationships with CBWF staff and other organizations. Good communication skills, such as listening well, and using language appropriately. Ability and willingness to provide emotional support, encouragement, and motivation to patients. Procures, coordinates, and monitors the delivery of needed services in accord with a service plan and identifies levels of need.
