Job Description:
Touchstone Communities is actively seeking a compassionate and
professional Licensed Social Worker to join our skilled nursing
team in providing exceptional care and support to our residents in
Texas. This role focuses on improving the emotional and
psychological well-being of our elders, ensuring their social needs
are met proficiently. As a member of our team, you will engage with
residents and their families to plan and implement personalized
care, collaborate with healthcare professionals, and uphold the
ethical standards of social work practice within a nurturing and
supportive environment. Assess residents' psychosocial needs and
develop individualized plans of care. Coordinate with healthcare
professionals to implement care plans effectively. Advocate for
residents’ rights and provide crisis intervention as needed.
Facilitate family and group meetings to discuss care strategies and
adjustments. Maintain comprehensive documentation of residents’
care and progress. Engage in continuous education and training to
improve professional knowledge. Strong interpersonal and
communication skills. Proficiency in case management software and
record keeping. Ability to handle sensitive information with
confidentiality. Skills in crisis intervention and behavioral
management. 401k Valid state-specific Licensure in Social Work (LSW
or equivalent). Certified Social Worker in Health Care (C-SWHC)
preferred.
